Waiver Statement

By completing this online transaction, I, the prospective client, being 18 years of age or older, or on behalf of my minor child, acknowledge that def SOL Productions and the instructors thereof, are not liable for personal injuries or loss of, or damage to, personal property. I understand that participating in dance classes involves the possibility of physical injury. I therefore assume all risks involved in the participation of dance classes at def SOL.
 

I hereby release def SOL Productions and staff from any liability with respect to damage or injury that I may suffer during participation in physical activity except where the damage or injury is caused by this gross or willful negligence of the staff within the scope of their duties.
 

I also give def SOL permission to use any photos or video footage of classes I may be in to promote their company or the hip hop culture. I understand that my email address will be used for communication purposes and will not be sold or misused.
 

def SOL reserves the right to make changes to the curriculum or schedule at any time. Registered participants will be notified. Classes require a minimum of 6 participants. If classes are cancelled due to low enrollment, registered participants will be notified via email and refunded within 24 hours.
